【背景知識】

胰島素樣生長因子（IGF）-l和-ll與血流中的特異性結(jié)合蛋白（IGFBPs）結(jié)合，目前已根據(jù)氨基酸序列鑒定出六種不同的結(jié)合蛋白，分為IGFBP-1，IGFBP -2，..IGFBP-6（第一個）。在血液中，IGFBP-3 占主導(dǎo)地位，主要用于調(diào)節(jié) IGF-1 和 -1I 濃度。與其他結(jié)合蛋白相比，IGFBP-3 除了結(jié)合 IGF-1 或 -II (3-5) 外，還具有結(jié)合酸不穩(wěn)定亞基 (ALS) 的特性。IGFBP-3 幾乎完全是這種形式高分子三元復(fù)合物，但血液中也有少量游離 IGFBP-3(1,2)。

開發(fā)出也可識別三元復(fù)合物中 IGFBP-3 的特異性免疫測定法為該蛋白的調(diào)節(jié)提供了新的見解 (2-5)。因此，IGFBP-3 被證明是診斷生長障礙的有用參數(shù) (2,3)。

然而，IGFBP-3濃度不僅受生長激素影響，還受其他因素影響：年齡和性發(fā)育、營養(yǎng)、甲狀腺功能減退、糖尿病、肝腎功能。營養(yǎng)不良時IGFBP-3值降低，甲狀腺功能減退、糖尿病和肝病 (1-3)。然而，在慢性腎功能不全的情況下，濃度會升高 (3,5,6)。24 小時測量顯示沒有晝夜波動 (7,8)。
